Yes - the box says that it's made in Japan

My ink feels like it has chunks in it- has anyone else experienced this, and can it be fixed?

I have not experienced it. My first thought would be to sent it back. If that were not possible, I would pour all ink out and try to strain the chunks out, fishing out what would not convert back to ink. FWIW

will the base of this bottle fit in an inkwell opening of 1 1/2 inches?

At the widest point I would say the bottle is about 3 in.

Is the color of this ink close to the blue in the pilot precise v5/v7 rollerball pens? i know this sounds like a silly question, but i like that blue.

As far as I know, the answer is no. Others you mentioned are blue-black(and all others not only Pilot). This blue is pure blue. Other companies sell the blue inks for fountain pens but they are all slightly different.
