Can you cut nose holes in it?

The fabric breathes really well, so it would not be necessary to cut holes in it. We also would not advise doing this because it would damage the fabric potentially and not provide full UPF 50+ protection.

Does it have to go over the head?

No, there are two snaps at back neck for adjustability.

does the UV protection come from chemicals or just the fabric?

It depends on the fabric. Zno is my favorite a cotton/bamboo fabriic infused with zinc or titanium. You'll have to read Coolibar's description for each type of fabric. The face masks are a blend of polyester and spandex.
