Do you have to take old toilet seat off?

If you have the standard round toilet, yes. If you have an elongated toilet (kind of egg shaped) it will fit with the toilet seat just raised. Hope this helps. Vikki

Does the inside of this seat "overhang" the inside of the bowl, to protect the lip of the bowl from being splashed (or worse...)? Thanks!

Can this be returned or exchanged?

Im pretty sure it can. Just be sure its very clean. Ive heard others say theyve received one that apparently had been tried but not very clean. To me thats so not right.

how is it fastened to toilet

The original toilet seat top has to be removed and thereby allows you to place correctly on toilet bowl and their are "fastens" on the sides. Very comfortable and with its own toilet seat cover also less unsightly to see.
