What are the sizes of the three connections on the y connector?

Sorry I didn't have to measure the size I just ordered OEM hoses and they fit perfectly.

Does the fitting at the y require hose clamps or is the plastic fitting at the y enough ?

The y is already clamped to the hoses. No clamps are required there. You will need clamps on the all the other ends that fasten to the heater, water pump and reservoir.

K1500 1998 does it work on ok older models

I don’t think so my 1999 C-1500 is definitely different.
