Not sure what the official guidance states, but my children have theirs running throughout the night, and we haven’t had any issues …

Do you put it directly below the area you want to project it onto?

No - the unit can be tilted, enabling you to adjust where you project to - e.g. ceiling or wall ..

Are the projections realistic?

Not as good as the Sega, but ok. The moon slide is very realistic, very detailed though.
