Q: Is this normal for the unit to have a loud pop when shutting down? it affects mon out lines and headphone output. TOS out is not affected, however.

I can hear a small blip in my monitors when I shutdown, but its not a loud POP. I use EXTREMELY sensitive in-ear monitors too, plugged directly into the headphone output. Not sure but its possible you might have a dirty power issue or some kind of low-voltage charge building up somewhere perhaps on a ground pin somewhere in your signal chain?

Q: Does this device come with a 15v DC power adapter?

Mine did come with the required DC power adapter in the original box.

Q: Are the On-board effects sent to the outputs or are they monitor only?

They can be real time monitor mix only with "pre fx" set to USB host OR sent to DAW as well with "post fx" set to USB host in input settings in Cuemix 5.
