Occasionally, I suffer acute lower back pain, which can be disabling. For decades I used a thick stretch-Neoprene product, designed for horse riders that worked brilliantly. It was broad, and had very thick side belts that could be pulled extra tight, but after 20 years, the Neoprene finally gave out.This item was my second choice for a replacement. The first was returned because not only was the size way off, it simply wasn’t substantial enough. This one is different. It is strong, the joints and stitching are substantial, and there is enough elasticity so you can use it gently, moderately or very tight. This is how it works - You have a main belt, with large Velcro pads. Wrap this tightly around your body. Then you use the two side bands to increase the tension from back to front, adding grip and support. It can be worn against the skin or over a T-shirt.For my needs, I want support from the top of my hips to just below my ribs. This belt is broad enough, and with a profile that keeps it in position. The four rigid rods (which you can’t feel when wearing), keeps it in position. The main belt has an elasticated strip on each side, so before you use the side-bands, you can fit the main belt quite tightly, if you wish. Then the side bands can tighten it to your taste. During serious pain, I need it very tight, and this belt is capable. The elasticated strips in the main belt are perforated, making perspiration less of an issue, but they are much thinner than my old Neoprene elastic, so we shall see if they last as long.Get a size that can be pulled very tight, even though you might not want that. I am on the cusp. I am 41.5” and I bought the Medium. I can fit my belt fairly tight, with margin to go VERY tight if I wish. Then pulling in the side bands really adds strength and support.After a week, my back pain subsided, first impressions are this is a well-designed and well-made product. I have concerns about the durability of the elasticated side panel fabric, but will need to wait and see. It’s dearer than some belts, but worth the money. I recommend it. Of course, if your pain persists, see a doctor.

Well-made. Easy to wear. Doesn't have patches of velcro poking your skin.I/m on the cusp between L and XL. When I misplaced the XL, I decided to order size L. Both sizes work for me. Obviously, I endorse this product.

I'd prefer more firmness but it was still better than no belt. It is NOT gym/weightlifting level belt though.

At 128 lbs (female) with a 30" waist a small is snug. Just fits. So if you want more leeway, pick the next size up. Been wearing it all day and feels good. Tweeked my back about a month ago, have some osteoarthritis in it, so I decided to look for a brace. So many choices to make, how do you choose and take a chance. This is good though. I noticed after the black one was a different shape and would have chosen that one if I noticed. Nevertheless, all good.
