Q: There's such a price difference, what's the main difference between this saw and the dewalt dw788?

Only difference is some of the outside sheet metal, the inner workings are the same. You are paying for the Dewalt name. They both used to owned by the same company.

Well worth it. I previously owned the dewalt 788 and they are practically the same but the delta is cheaper in price. Im a fan

Q: The saw is great but I can not get the hold down to stay in place. I move the wood and it also moves. Any thoughts.

I removed mine even though it held just fine as I found that it got in my way. This was after I'd gotten used to the saw and the forces involved. I haven't had an issue using it without the hold down, but you have to do the holding down.

Q: What is the 'thing' on left side? How easy to change blade, need a tool? Disappointed no light, any available?

That black tube thing on the left is an adjustable hose that blows puffs of air (generated I think from the up and down motion of the upper arm) which will help keep your lines clear of sawdust. Changing blades is a snap, simply loosen the thumb / wing screw on the top arm and the lower holder and slide out the blade. As for the light, I make due with a shop lamp that I have hanging from the ceiling of my garage. You can check woodcraft or rockler for magnetic portable lamps that may stick to the saw somewhere - not sure if its magnetic everywhere, never tried
