This is the worst form I’ve ever seen. It was supposed to include everything and the only thing it includes is if you want nutrition and hydration, it does not include DNR or DNI do not purchase.

Simple to understand.
This is easy to understand. It has forms that are premade with blanks to fill in your information. You fill in the blank areas and then have it notarized. I’m not a lawyer. I do not how much legal standing it has in the world of courts and laws. I feel like it is a basic form that will allow others to know your wishes in an organized manner.
