We suggest consulting a professional repair tech to verify if the amp is the problem or if the tube is bad. However, shipping dimensions would vary depending upon how the supplier decides to package the tube for shipment. It has been my experience that this is inconsistent and nearly impossible to predict. Suppliers typically package based on what will be sufficient to protect the item in transit and what materials they happen to have on hand. So, you might order from the same company 2 times and the shipping materials could be different. Q: Would there be further volume reduction if I also replaced the second preamp tube?

Since a 12AT7 has only 60% of the gain of a 12AX7 yes it would reduce preamp output and volume at that stage.
