The intensity has three levels of adjustment. To pin-point an area you can run it in manual mode (which puts it in one position) or in this mode they call BodyMap Pro which works one area. I don't really know what shiatsu is so I can't answer that one. I can tell you that I've used mine everyday for probably 2 years and can't imagine being without it.

How much assembly is required?

I'm 6'1" and it comes up high enough to massage your neck. There are range adjustments you can make on the hand controller to dial it in pretty precisely.
